38岁包皮是否需要割除的问题，主要取决于是否存在包茎、包皮无法上翻、包皮过长合并反复感染等情况，如果存在以上情况，38岁仍然可以进行包皮手术，因为包皮问题容易引发感染，且感染风险不会随着年龄增加而下降。
